On 19 April 2017, NOPSEMA’s issued Esso Australia Pty Ltd an Environmental Improvement Notice for failing to follow the procedures for obtaining a representative sample of spilt oil in accordance with their accepted environment plan.

NOPSEMA has published a safety alert following an incident where two collared eyebolts failed during lifting operations resulting in a 21.7 tonne diverter falling up to 1.8 metres to the facility floor.

A recent NOPSEMA inspection identified maintenance on safety-critical equipment was not carried out consistent with commitments made in the facility safety case.
